Available Items to Add to a Page
This is a list the available items that can be added to a Forms page. Each item has specific rules and options and these are also described here. The items are listed in alphabetical order. The Add Item to a Form page allows you to select the type of item for the form.

Checkboxes allow you to gather multiple answers to the same question. You can have a specific answer checked by default and specify the minimum and maximum number of answers the user can choose. If preferred, you can randomize the order in which the answers are displayed and optionally add number labels for each answer.
Checkbox fields:
Form Options - Number answer inputs
Use this field to determine how many answers you want to display. The
default is 10. You can change this amount and then click the Change
button to have the page display a different amount of answers.
Question/SubText - Question Text
Enter the question in this text box.
Question/SubText - Sub Text
Use this field for any instructional or other text.
Enter Answers - Answer Text
This field is used for an answer to the question. You should enter all
the possible answers to the question in these text boxes.
You can also determine which checkbox(s) should be checked by default. If
you want an answer(s) defaulted, click the Checkbox (in the Checked? Column)
next to the answer.
Enter Answers - Cost
If there is a cost associated with the answer(s), enter the cost in the
Cost text box. You should enter costs were applicable for each answer.
All added costs are summarized by registrant or by event, whichever is
applicable, and shown to the user before completion of the registration
process. The adjusted cost inclusive of all adds and subtracts from the
base cost, as defined in the event setup, cannot be less then $5.00 for each
registration
Enter Answers - Same Answer As
This gives you the option of selecting the same answers that have been
set up for another question in the form. You can select the question from
the drop-down list provided and the system will copy those answers and paste
them for this item.
Other Options - Allow �other� and Other text
You have the option of have an answer of Other in your list of answers. If
you choose this, you can also enter a heading for the Other text box (in the
�Other� text field).
Other Options - Randomize Answer Order
You can opt to have the answers appear in a different order each time
the form is retrieved by a user. This can be done in order to avoid having
the same answer receive more attention by being at the top of the list.
Other Options - Number Labels
You can assign the answers number labels with this option.
Other Options - Maximum to Choose
Enter the number of maximum number of answers that you want the users to
select.
Other Options - Minimum to Choose
Enter the minimum number of answers that you want the users to select.
If you want the users to always answer this question, enter at least 1 in
this text box.
This allows uses to enter a date value on the form. You can specify default date, first date allowed, last date allowed, and indicate if a date must always be selected by the user.
Date fields:
Question/SubText - Question Text
Enter the question in this text box.
Question/SubText - Sub Text
Use this field for any instructional or other text.
Required - Answer is Required
If you want the user to always answer this question, click the Answer is
Required checkbox. If it is an optional question on the form, leave this
checkbox blank.
Item Options - Default Date
This item is the date that will default on the form. Choose the
month, day and year from the drop-down lists.
Item Options - Earliest Date Allowed
This date will be the earliest date that the user can choose.
Select the month, day and year from the drop-down lists.
Item Options - Last Date Allowed
This is the last date that the user can choose. Select the
month, day and year from the drop-down lists.
A dropdown Menu is the same as a drop-down list. These items allow the users to choose from multiple answers without taking up much room on the page. These are especially useful for questions with a large number of potential answers, or pages with a large number of items. You can specify the default answer, and if an answer is required for the question. If preferred, you can randomize the order in which the answers are displayed and optionally add number labels for each answer.

This field is used for an answer to the question. You should enter all
the possible answers to the question in these text boxes.
You can determine which answer should be defaulted when the user clicks the
dropdown menu. Click the radio button (in the Default? Column) next to the
answer that you want to default.
Enter Answers - Cost
If there is a cost associated with the answer(s), enter the cost in the
Cost text box. You should enter costs were applicable for each answer.

Other Options - Number Labels
You can assign the answers number labels with this option.
This item will display plain text as a header. The Header Text is displayed in large letters, and the Sub Text is displayed in smaller letters.
Header fields:

Enter the text for the Header in this text box.
Header Options- Sub Text
Enter any sub text you want for the header in this field.
This option displays a horizontal line across the page. There are no options for this item.
This item allows you to add freeform HTML to a page.
HTML fields:
HTML
The text box displayed for your HTML entry will vary. If you are using
a version of Internet Explorer greater than 5.5, the
Rich Text Editing text box will be displayed. Please refer to those
instructions for using the text box.
If you are using another Browser or a version of IE less than 6.0, a normal
text box is displayed.
This allows you to identify an image to be displayed in the form. You can specify an image on the server, or upload an image from your hard drive.
Image fields:
Image Options - URL
You can either enter a URL of where the image is stored on the Web or if
you need to upload an image, you can use the Upload button to perform
that function. When you use the Upload button, a new page will be displayed
so that you can Browse your hard drive and select the file to be
uploaded.
Message text is displayed in bold red letters on the form.
Message fields:
Message
Enter the text that you want displayed on the form page in the Message
text box.
This option allows you to gather freeform text from users filling out the form.
Multiple Line Text fields:

Enter the question in this text box.
Question/Sub Text - Sub Text
Use this field for any instructional or other text.
Required? - Answer is Required
Check this checkbox if you want to require that the user answer this
question in order to complete the form.
Item Options - Default Value
This is the text box that will be displayed on the form. If you want some
default value to be displayed, enter it in this box.
This item allows the user to choose one answer from a list of choices. These are ideal for questions with a small number of answers. You can also allow the user to choose Other. You can have a specific answer selected by default. If preferred, you can randomize the order in which the answers are displayed and optionally add number labels for each answer.

Determine how you want the radio button answers to be displayed on the
form - either horizontally or vertically on the page.
Use the single line text field to allow the user to type in a freeform response.
Single Line Text fields:

Enter the question in this text box.
Question/Sub Text - Sub Text
Use this field for any instructional or other text.
Required? - Answer is Required
Click this checkbox if the user must answer this question.
Other Options - Required Answer Format
You can specify if there is a format that is required for the answer.
Numbers, Integers, Decimals, or Money are the format choices. If there is
no required format, leave None selected in this field.
Other Options - Minimum Value
If there is a format and you have a minimum value that should be entered
by the user, enter that minimum value in this field.
Other Options - Maximum Value
If there is a format and you have a maximum value that can be entered by
the user, enter that maximum value in this field.
Other Options - Default Value
If you want a default value displayed in the single line text field,
enter it in the Default Value text box.
