Inquiry Contact, Address and Telephone

This information appears on the confirmation receipt sent upon completion of a dues payment or commitment.  It is provided in the event there is a need to contact the organizer.  The organization name is automatically added to the confirmation receipt and cannot be changed.  The Contact EMail field is added as well.

The contact, address and phone information is pre-entered based on the sign on user.  It can be overridden by editing the fields.

This information is required when collection of online dues is enabled.
